Medical Bill Letter Builder

In short

The Bill letter builder writes four common letters about a medical bill. You can ask for an itemized bill, ask for financial assistance or a lower bill, or dispute a balance bill. You can also tell a provider you started a No Surprises Act dispute. Fill in your details, then download, print or copy the letter. Nothing you type is sent.

How to use the builder

  1. Choose the letter. Itemized bill, financial assistance, balance bill, or the No Surprises Act dispute note.
  2. Tick what applies. For example, whether you have insurance or Medicare.
  3. Fill in your details. Use the numbers from your bill and, if you have one, your Explanation of Benefits.
  4. Download, print or copy. Send it to the billing office in the way the bill asks, and keep proof you sent it.

Which letter do I need?

Frequently asked questions

Is anything I type saved or sent?

No. The letter is built in your browser. Nothing is sent to Aptiva or anyone else.

Can a letter stop collections?

A letter can ask the provider to hold your account while they review it, but it doesn’t guarantee that.
